Due to the current weather forecast, all Baton Rouge Clinic locations in the Greater Baton Rouge Area will be closed all day Tuesday, 1/21 and Wednesday, 1/22. 

This includes our locations in Opelousas, New Roads and Prairieville as well as our Mid City Medicine Clinic.

Our Urgent Care Center in Baton Rouge will be closed on 1/21, but will open on 1/22 once it is safe to do so.

Medicare is changing their reimbursement model and adding a new program called Advanced Primary Care Management.  This program is designed to enhance the care we provide to you by focusing on prevention, coordination, and overall well-being. By enrolling in this program, we will be able to dedicate more time and attention to understanding your unique health needs and offer a wide range of services to meet these needs. This new program includes new CPT codes, which were designed to cover the expenses of these services.

As a Baton Rouge Clinic patient, you may already be receiving the benefits of some of these services, but it is important to note that with these new codes, the reimbursement for the regular evaluation and management codes is being reduced.

These services will include:
      • 24/7 access to your care team for urgent needs.
      • Communication through MyChart.
      • Offering Telehealth visits, when deemed necessary.
      • Coordination of care with other providers and specialists.
      • Access to your individualized care plan developed by your care team.
      • Scheduling routine appointments to monitor all health maintenance and your care plans’ progression.
      • Ensuring that you are up-to-date on preventative services (i.e. routine immunizations), medication management, and assessing your medical and psychosocial needs.
      • Various health reminders for preventative services like flu shots, colonoscopies, etc.

As always, we are committed to providing the highest level of quality healthcare services for all of our patients. These services are meant to ensure you receive the best possible care and support from your care team.

Please note that we will be filing these new charges to Medicare, and Medicare is stating these charges will be filed towards your preventative healthcare benefits.
