Please have your insurance card available when making your appointment. Many managed care insurance plans require a primary care physician referral prior to making a specialist appointment to ensure payment of full benefits. After Your Appointment is Scheduled

All new patients are encouraged to download the Patient Registration Form ahead of time. New patients can print and complete the form and then bring it with them to their next visit in order to lessen time spent in the waiting room.

It is also advisable to bring either all of your medications or a complete list of all of your medications, along with the dosage and frequency you take them. The list should be complete and include all prescribed and non-prescribed medicines, regardless of the recommending or prescribing physician.

If you are unable to keep your appointment, please call as early as possible to cancel or reschedule.

Check-In Process for Patients

To comply with social distancing, we are checking in patients while they wait in their vehicles. Upon arrival at the clinic please call (225) 246-9009 and a registration clerk will assist you with verifying your information and checking you in.
