Please have your insurance card available when making your appointment. Many managed care insurance plans require a primary care physician referral prior to making a specialist appointment to ensure payment of full benefits. After Your Appointment is Scheduled

All new patients are encouraged to download the Patient Registration Form ahead of time. New patients can print and complete the form and then bring it with them to their next visit in order to lessen time spent in the waiting room.

It is also advisable to bring either all of your medications or a complete list of all of your medications, along with the dosage and frequency you take them. The list should be complete and include all prescribed and non-prescribed medicines, regardless of the recommending or prescribing physician.

If you are unable to keep your appointment, please call as early as possible to cancel or reschedule.

Check-In Process for First-Time Patients

If it is your first visit to the Baton Rouge Clinic, please stop at the front desk for assistance in locating your physician’s check-in station. There are also signs in the building to assist you in finding your physician.

All patients need to check in with the receptionist on the appropriate floor so that we know you have arrived for your appointment. The receptionist will verify your insurance information at each visit, so please bring your insurance card and notify the receptionist of any changes in your coverage. The receptionist will also collect any co-payments or portions of your bill that are not covered by your insurance for that visit. He or she will also assist you in scheduling your next appointment.
