Please have your insurance card available when making your appointment. Many managed care insurance plans require a primary care physician referral before making a specialist appointment to ensure payment of full benefits. After Your Appointment is Scheduled

All new patients are encouraged to download the Patient Registration Form ahead of time. New patients can print and complete the form and then bring it with them to their next visit to lessen the time spent in the waiting room.

It is also advisable to bring either all of your medications or a complete list of all of your medications, along with the dosage and frequency you take them. The list should be complete and include all prescribed and non-prescribed medicines, regardless of the recommending or prescribing physician.

If you are unable to keep your appointment, please call as early as possible to cancel or reschedule.

Check-In Process for Patients

To maintain social distancing, we have implemented an optional remote check-in procedure.  When you are seeing your physician in the clinic you can remain in your car upon your arrival and call the check-in desk at (225) 246-9009. They will verify your information and check you in while you are in your car waiting.  The registration clerk will get your contact number so that the nurse can call you when the doctor is ready to see you.
